Voter Identification Card is the basic and only requirement for a citizen of India to perform his most important duty as a citizen of India and that is casting a vote. - Duplicate Voter identity card is issued only if the previous one is lost.
- File an FIR (First Information Report) of your lost voter id card in the nearest police station and get that FIR photocopied.
- After filling the form attach the photocopy of the FIR along with the form and go to the nearest Electoral Registration Office.
- Along with the Duplicate voter ID card form attached to the FIR a penalty of RS 25 has to be submitted and along with it bring the other proof of identity like PAN card or driving license and address proof like your bank pass-book or electricity bill.
