About Form-6:

Who can apply for Form-6?

  • Applicants who attain age of 18 years or more on the first day of January of the year with Reference to which the Electoral Roll is being revise can file Form-6.
  • Person who is shifting his/her place of ordinary residence outside the Constituency in which he/she is already register can also file Form-6.

When Form-6 can filed Process?

  • Any applicant can filed after the completion of draft publication of Electoral Roll of the Constituency. Remember the form is to filed within the specific days provided for the purpose.
  • Because publicity is given about the above period when the revision programme is announced.
  • An applicant is to filed only one copy of the application.
  • If a person want to include the name, then it can filed throughout the year even when the revision programme is not going on.
  • During the non-revision programme applicant must be filed in duplicate.

How to fill the Form-6?

  • To apply the form-6, you should be addressed to the Electoral Registration Officer at the Constituency where you seek registration. The name of the Constituency should be mentioned in the required field.
  • Your name with documentary proof. The name should be appear same in the Electoral Roll and Electors Photo Identity Card. The full name without including the surname should be written in the first box and surname should be written in the second box. If you don’t have the surname just write the given name. You need to mention your caste only in the caste name field and not anywhere else as it is used as part of the Elector’s Name or a Surname.
  • You should not mention honorific appellations like Shri, Kumari,Smt, Begum, Khan, Pandit, Ms, Mrs, Mr, etc.

About Form-6A:

Who can apply for Form-6A?

Applicant who want an inclusion of name in Electoral Roll by an overseas Indian elector can apply for Form-6A. Remaining procedure is same as Form-6.

About Form-7:

Who can file Form-7?

This form can be filed only by a person whose name is already included in that Roll in order to object the inclusion of a name proposed to be included in that part of the Electoral Roll or for seeking deletion of a name which is already included in that part of the Electoral Roll.

When Form-7 can be filed?

  • Any applicant can be filed after the completion of draft publication of Electoral Roll of the Constituency. Remember the form is to be filed within the specific days provided for the purpose.
  • Because publicity is given about the above period when the revision programme is announced.
  • An applicant is to be filed only one copy of the application.
  • The applicant seeking for deletion of an entry from the final Electoral Roll can be filed throughout the year even when the revision programme is not going on. During no revision period, application must be filed in duplicate.

How to Fill the Form-7?

  • To apply the form-6, you should addressed to the Electoral Registration Officer at the Constituency in which another elector registered registered in that roll objects the proposed of an entry in the draft Electoral Roll.
  • The name of the Constituency should mentioned in the required field.
  • Details of the person inclusion of whose name is objected to or the details of the person whose entry is to deleted.
  • Out of these two alternatives, the first option is relevant after draft publication of Electoral Roll during the revision of Electoral Roll. The second option is relevant during continuous updation even after final publication of Electoral Roll.
  • Applicant also need to fill the Serial No of the entry in that part of Electoral Roll and Identity Card No. which is issued to that person.
  • The part no. of the Electoral Roll is printed on the right hand top corner of the number at which the name of the person whose entry is objected for inclusion/sought to be deleted is listed.
  • Separate application is required to be filed for objecting the inclusion or seeking the deletion of each entry.

About Form-8:

Who can apply for Form-8?

Applicant whose name is already included in the Electoral Roll seeking correction to his/her particulars which are already printed in the Electoral Roll can file Form-8.

One person cannot file application in Form-8 for correction of any other person in the particulars.

When Form-8 can filed?

  • This form can filed for the correction of particulars of a person entered in the Electoral Roll. The application can filed after the completion of draft publication of Electoral Roll of the Constituency.
  • The application is to filed within the specific days provided for the purpose. Due publicity is given about the above period when the revision programme is announce.
  • An applicant is to filed only one copy of the application.
  • The applicant can also filed for correction of particulars relating to oneself in the finally published Electoral Roll when the revision programme is not going on.
  • During the non-revision programme applicant must  filed in duplicate.

How to Fill the Form-8?

  • The application must be addressed to the Electoral Registration Officer of the Constituency in which the name of the applicant is already included.
  • The name of the Constituency need to mentioned in the required field.
  • In Part-I of the application, write your name, as it should be appear in the Electoral Roll. If you want your initial to printed in the expanded form, then you can writer your full name with expanded initial.
  • And You should write your full name in the first box and the surname in the second box. You should mention your caste only in the required field.
  • You should not mention honorific appellations like Shri, Ms, Smt, Pandit, Khan, Mrs, Mr, Begum, etc
  • It is mandatory to full up the part number and serial number in that part of the Electoral Roll where your name is list.

About Form-8A:

Who can file Form-8A?

This application can filed for transportation of an entry from one part of electoral roll to another part of electoral roll of a constituency. Remaining procedure is same as filing the application of Form-8.
